RN Care Coordinator – BPCI / Orthopedic Focus

Full-Time | Day Shift | On-Site | Mississippi Gulf Coast

We are seeking an experienced RN Care Coordinator (LPN with 10+ years considered) to manage patients through a 90-day episode of care under the BPCI Advanced and other bundled payment programs. This role focuses on orthopedic patients and care coordination across the full continuum of care.

Key Responsibilities:

• Manage patients throughout a 90-day bundled care episode

• Complete pre-op risk assessments and post-op clinical evaluations

• Develop and implement individualized care plans

• Coordinate care across acute, post-acute, rehab, SNF, LTAC, home health, and outpatient settings

• Arrange services including home health, DME, PT, and community resources

• Provide telephonic and onsite follow-up

• Maintain accurate EMR documentation

• Identify and address care gaps and barriers

• Ensure HIPAA compliance

Qualifications:

• Active Mississippi RN license 

• LPN with 10+ years of experience considered

• 2–3 years nursing experience required

• Orthopedic experience highly preferred

• Discharge planning and outpatient care coordination experience preferred

• Strong EMR, documentation, communication, and time management skills

• Ability to work independently in a fast-paced environment
