    Home Care Occupational Therapist Direct Hire – Creston, IA

    $35.67 to $53.50 per hour, based on years of experience.

    Purpose:

    Provides comprehensive Occupational Therapy services to individuals in the home setting for the purpose of increasing and maximizing functional abilities to perform daily self-care tasks, accepting primary care responsibilities, and coordinating with other disciplines and community agencies.

    Responsibilities:

    • Evaluates/assess the patient/client and determine a treatment plan using approved examination and evaluation procedures.
    • Interprets evaluation findings and communicate the treatment plan to the patient/client, family, caregiver, and care team.
    • Prepares necessary documentation including evaluations, daily notes, progress notes, updated plans of care, discharge summaries and logs in a timely manner according to federal, state and facility-specific rules and regulations. 
    • Consults with other members of the rehabilitation team to select treatment programs consistent with the needs and capabilities of the patient and to coordinate comprehensive services delivery.
    • Develops effective home programs to maintain and enhance patient/client performance in his/her environment.

    Job Requirements

     

    • Bachelor’s Degree in Occupational Therapy
    • Current and valid Iowa OT license
    • Ability to comply with accepted professional standards and ethical guidelines
    • Current Healthcare Provider CPR certification required
    • Mandatory Reporter of Child and Dependent Adult Abuse training certification (within 30 days of hire)
    • Must have a valid driver’s license, auto liability insurance, and reliable transportation
    • Successful completion of pre-employment physical, drug screen, 2-step TB test, and background check

    HUGE benefit: FLEXIBILITY in schedule to allow a work-life balance 

    Hourly rate: $35.67/HR - $53.50/HR mileage reimbursement rate 64 cents/mi (highest allowed federal rate) starts from 1st destination - NOT taxed. Clock-in first location - Clock-out last location.

    Health ins eligible 1st of the month following hire
     
    SCHEDULE: 
    Typically M-F 8-5: based on patient needs and schedule (flexible!) No On-call
