Title: MDS Coordinator
Location: Honolulu, HI
Pay: Up to $85k/yr (DOE)

MDS Coordinator Responsibilities:
Ensure the completion of all MDS assessments and required documents are completed and compliant
Create a schedule for all Medicare and Medicaid.
Start Medicare coverage for newly qualified patients or send out denial letters.
Updated on changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.
Ensure that all medical records are fully completed and accurate
Assists DON, ADON, social Services, and Administrator in developing and implementing patient care plan

MDS Coordinator Requirements:
BSN from an accredited school
Hawaii State RN license
CPR/BLS and First Aid Certification
Must have a minimum of 1 year of MDS experience and patient-driven payment models (PDPM) experience
Minimum 3 years of charge nurse in a Long Term Care setting; preferred
Knowledgeable in CMS guidelines
