Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background and the ability to work independently while collaborating with a multidisciplinary team. The RN will be responsible for assessing patient needs, developing care plans, and delivering direct patient care.
Responsibilities
- Conduct comprehensive assessments of patients’ health status and needs in their home environment.
- Develop and implement individualized care plans based on patient assessments and goals.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patients’ progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education to patients and families regarding health management, disease prevention, and rehabilitation.
- Maintain accurate and timely documentation of patient care activities in compliance with regulatory standards.
- Collaborate effectively with other healthcare professionals, including physicians, social workers, and therapists, to coordinate patient care.
- Participate in quality improvement initiatives to enhance the standard of care provided.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health care or similar settings is preferred.
- Knowledge of nursing practices related to MDS (Minimum Data Set) assessments and inpatient care is advantageous.
- Skills in performing procedures such as spinal taps or gastric lavages are a plus.
- Experience working with children or providing dementia care is highly desirable.
- Familiarity with nursing home environments and practices is beneficial.
- Understanding of anatomy knowledge relevant to patient assessments and interventions.
- Certification or training in dermal fillers is a nice-to-have but not required.
