Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide a comprehensive assessment of the patient's needs, development of a care plan, and education of the patient and family on the plan of care. . This role requires a strong clinical background and the ability to work independently while collaborating with a multidisciplinary team. The RN will be responsible for assessing patient needs, developing care plans, and delivering direct patient care.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and needs in their home environment.
• Develop and implement individualized care plans based on patient assessments and goals.
• Administer medications, treatments, and therapies as prescribed by physicians..
• Provide education to patients and families regarding health management, disease prevention, and rehabilitation.
• Collaborate effectively with other healthcare professionals, including physicians, social workers, and therapists, to coordinate patient care.
• Participate in quality improvement initiatives to enhance the standard of care provided.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health care or similar settings is preferred.
• 1 Year of Oasis assessment experience
• Familiarity with nursing home environments and practices is beneficial.
