This role includes case management responsibilities and supervision of Home Health Aides (HHAs) and Physical Therapy Assistants (PTAs), ensuring care is delivered in accordance with best practices and regulatory standards.

Physical Therapist Key Responsibilities:

• Evaluate and admit patients who meet agency and regulatory criteria.
• Develop and implement personalized treatment plans based on patient needs.
• Conduct ongoing reassessments and discharge planning.
• Create and delegate care plans for HHAs and PTAs; supervise and provide feedback per agency policy.
• Serve as case manager for assigned patients, coordinating with internal and community resources.
• Deliver patient care tailored to medical condition, age, and developmental status.

Physical Therapist Qualifications:

• Graduate of an accredited physical therapy program recognized by NBCOT.
• Valid State of Hawaii Physical Therapist license.
• Current BLS certification (American Heart Association).
• Current medical/health screening including 2-step TB clearance.
• Minimum 1 years of experience as a Physical Therapist, preferably in home health or a healthcare setting.
• Must have a valid state of Hawaii driver's license and a clean driver's abstract

Salary Range: $105,000 - $110,000 per year depending on experience + competitive benefits!
