We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care to patients across various settings, including nursing homes, medical offices, and memory care facilities. Duties

• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report any significant changes to the healthcare team.
• Provide personal care assistance to patients, including bathing, dressing, and grooming.
• Assist in the development and implementation of patient care plans.
• Educate patients and their families about health management and wellness practices.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Participate in hospice care for terminally ill patients, providing comfort and support.
• Engage in toddler care when necessary, ensuring a safe environment for young patients.
• Utilize sonography techniques as needed for patient assessments.

Qualifications

• Valid LPN license in the state of practice.
• Experience in geriatrics, nursing homes, or memory care preferred.
• Familiarity with pediatric nursing or experience caring for children is a plus.
• Strong interpersonal skills with the ability to communicate effectively with patients and families.
• Compassionate nature with a commitment to providing high-quality patient care.
• Ability to work collaboratively within a health care team environment.
• Knowledge of basic medical office procedures is advantageous.
• Experience in hospice care is highly desirable.
